What is a multiple awards plate?
The design of this plate will allow you the opportunity to display the number of awards received of this type. If you received 2, 3, 4, or more awards, the number will be displayed in alpha format on your plate (i.e., TWO AWARDS, THREE AWARDS, FOUR AWARDS, etc.)
Must provide copy of certificate of award (or) copy of official records (or) letter showing medals were awarded (or) copy of DD214

One free registration per qualifying applicant (emissions fee and personalization fees must be paid, if applicable). Unremarried surviving spouses of qualifying applicants are eligible to receive these license plates.
